The Behavioral Health Home (BHH) is a comprehensive care program designed to support your physical and mental health. By acting as your central point of contact, the BHH team ensures all aspects of your healthcare work together seamlessly.

 

Program Overview

The BHH program is built on the foundation of providing holistic, empathetic, and person-centered support. Our approach focuses on treating the whole person by attending to complete health and social needs.

  • Integrated Care: We bridge the gap between behavioral and physical health by coordinating care across the broader healthcare system, ensuring you receive the best care possible.
  • A Specialized Team: You will work closely with a dedicated interdisciplinary team, which includes a case manager, therapist, psychiatrist, and nurse. This core team is further supported by a medical assistant and a community health worker.
  • Holistic Wellness: Our team provides empathetic, person-centered support to attend to your complete health and social needs.
  • Voluntary Participation: Enrollment in the Behavioral Health Home is entirely your choice, and you may opt-out at any time.

Services Provided

Our program provides six essential services to ensure you receive the best care possible:

  1. Comprehensive Care Management: Total oversight of your health journey. Your team works with you to create a personalized plan, set goals, and address physical, behavioral, and recovery needs.
  2. Care Coordination: We assist with scheduling appointments, arranging transportation, and managing referrals to specialists. We also help you obtain medical equipment, such as blood pressure cuffs or glucometers, to manage your health at home.
  3. Health Promotion: We provide the education and resources you need to build healthy habits, such as improving nutrition or quitting smoking.
  4. Comprehensive Transitional Care: Dedicated assistance when moving between care settings, such as returning home after a hospital stay.
  5. Individual and Family Support: We provide resources and support systems to help both you and your loved ones navigate your care.
  6. Community & Social Referrals: Direct connections to vital social services, housing assistance, and community programs.

Eligibility & Enrollment

To participate in the ACCESS Behavioral Health Home, individuals must meet the following criteria:

  • Insurance: Must be a Medicaid beneficiary.
  • Location: Specifically serving clients within the Detroit Wayne Integrated Health Network (DWIHN).
  • Provider: Must be an active client receiving services through ACCESS.
